The DOT requires up-to-date Driver Qualification Files (DQF), which include:

  • Medical Exam Certificate & Verification forms
  • Copy of CDLs
  • Complete Driver Application
  • MVR (reviewed every 12 months)
  • Road Test Form & Certificate
  • PSP (record of violations)
